Pension Forms:


	Notification of Retirement Form Notify Simplot of your intent to retire. This form should be completed 90 days prior to your intended retirement date.

	Request for a Pension Calculation Ask the Employee Service Center to calculate a pension estimate. Please allow 4-6 weeks turnaround.

	Pension Payment Options Quick sheet lists the different pension payment options available for term vested retirees.
